Senior Case Management Coordinator

Commence: ASAP - 3 months with possible extension
Rate: $56.05p/hr + Super
Location: CBD

Our client is looking for a strong client advocate to join them in their mission to make a profound difference in the lives of veteran families!

As the Senior Case Management Coordinator, you will be providing support veteran families. Your role will involve conducting thorough assessments, crafting personalised support plans, coordinating essential referrals, and passionately advocating for the wellbeing of our clients. You will work hand-in-hand with the Community Services Team Leader, internal teams, volunteers, and external service providers to deliver timely and effective support. Additionally, you will collaborate closely with the Community Services Program Specialist and Executive Officer to enhance service offerings and frameworks, aligning them with best practice.

Key Responsibilities:
  • Conduct comprehensive assessments to understand and address the unique needs of veteran families.
  • Develop and implement tailored support plans that make a real difference.
  • Coordinate referrals to a network of internal and external service providers.
  • Advocate tirelessly for the wellbeing of veteran families.
  • Foster strong collaboration with internal teams, volunteers, and external partners.
  • Contribute to the continuous improvement of our service, scope, and risk frameworks.
Key Selection Criteria:
Essential
  • Qualification in Social Work, Psychology, Community Services, or a related field.
  • 2-3 years’ in case management, preferably with vulnerable populations.
  • Demonstrated understanding of trauma-informed care and person-centered support.
  • Exceptional communication, collaboration and stakeholder engagement skills.
  • High-level organisational skills and ability to manage multiple cases simultaneously.
Desirable
  • Experience working with veteran families, mental health services, or financial support programs.
  • Lived experience in the veteran community.
  • Experience working with First Nations communities.
  • Mental Health First Aid, ASIST and/or SafeTALK certificate or willingness to obtain.
  • The Advocacy Training and Development Program (ATDP) accreditation or willingness to obtain.
